Combination inhalers, which contain both an ICS and a long-acting beta-agonist (LABA), simplify treatment regimens by delivering both controller and reliever components in a single device. Biologics represent a major therapeutic advance for patients with severe, uncontrolled asthma. These targeted therapies, administered via injection or infusion, block specific inflammatory pathways, offering a new level of disease control for patients who do not respond to conventional therapies.

Controller medications, particularly inhaled corticosteroids, form the cornerstone of therapy for persistent asthma. They work by reducing inflammation in the airways, decreasing airway hyperresponsiveness and preventing symptoms. For patients with moderate-to-severe asthma, a combination of an ICS and a long-acting beta-agonist (LABA) is often recommended. LABAs are bronchodilators that provide sustained relaxation of airway smooth muscle, complementary to the anti-inflammatory effects of ICS. Several fixed-dose combination inhalers are available, simplifying therapy and improving adherence. For patients with severe asthma not controlled by high-dose ICS/LABA therapy, biologics offer a new therapeutic avenue. These medications target specific molecules and immune cells that drive airway inflammation, such as anti-IgE for allergic asthma or anti-IL-5 for eosinophilic asthma. The clinical impact of biologics has been substantial, offering significant reductions in exacerbations and improvements in lung function and quality of life.
